For your convenience, all of our New Patient forms are available to you online. Please complete the Registration, Dental and Medical History, and HIPAA Consent forms. Some forms require a digital signature and please do not forget to hit the Submit button in the upper left-hand corner of the screen. Our Notice of Privacy Practices is posted here for your review, and the Authorization of Dental Records and Radiographs is in a printable format for your use.

First Visit at Beverly Restorative and Implant Dentistry Center


Becoming a new patient is easy. Call our patient care coordinators to set up your first visit today.

To help us better serve you, please bring the following to your first visit:

  •     Dental insurance card and any secondary dental insurance information
  •     Any current dental radiographs (see note below on)
  •     An updated list of any medications and supplements you may be taking
  •     If you are currently under the care of a physician, please bring their contact information

Please alert Dr. Lee or the office prior to your appointment if you have a medical condition or require antibiotics prior to dental cleanings. A parent or guardian must accompany all patients under the age of 18 during the first visit.


Our office is open Monday, Thursday & Friday 8:00-5:00, Wednesdays 11:00-7:00, Tuesdays and Saturdays can be booked by appointment.

Insurance and Financing

We believe that quality dental care should be available to everyone. If you have dental insurance, we will be happy to submit a pre-treatment estimate or dental claim on your behalf.

Dental Payment Plans Beverly Restorative and Implant Dentistry Center offers two commercial financing options; CareCredit and Lending Club. In addition to these options, Beverly Restorative and Implant Dentistry Center can arrange payment plans, please consult with the front desk regarding these options.

Dental Records

Radiographs are an important part of a comprehensive exam. To avoid unnecessary delay of treatment and unnecessary additional radiation exposure please have any recent radiographs forwarded to our office in advance. Bitewing radiographs within the past one year and Full Mouth Series or Panorex radiographs within the past five years are acceptable.
