First Visit with Dr. Lee
For a cleaning and checkup you will see one of our dental hygienists. The hygienist will do a comprehensive evaluation of your teeth and gums followed by tooth scaling and polishing. Then Dr. Lee will see you for the dental exam.
For those with specific dental concerns we offer a preliminary consultation to discuss your needs and to determine your personal goals and expectations of treatment. Occasionally, minor treatment can be done the same day. Diagnostic data collection may be necessary to create a personalized treatment plan for you. This may include radiographs, diagnostic models and digital photographs. This additional information will then be analyzed to formulate a treatment plan and to estimate cost prior to the start of any treatment.
To help us better serve you, please bring the following with you at your first visit:
- Referral form if applicable
- Dental insurance card and any secondary dental insurance information
- Any current dental radiographs (see note below on current radiographs)
- An updated list of any medications and supplements you may be taking
- If you are currently under the care of a physician, please bring their contact information
You may print the Patient Registration Form and Dental and Medical History Form at home. Please fill it out at your leisure and bring it with you to your first appointment. This form is also available at the bottom of this page.
Please alert Dr. Lee or the office prior to your dental appointment if you have a medical condition or if you require antibiotics prior to dental cleanings.
Please note that a parent or guardian must accompany all dental patients under the age of 18 during the first visit.
Radiographs are an important part of a comprehensive exam. We ask that you please have any recent radiographs forwarded to our office in advance, or that you bring hard copies to us the day of your appointment. This avoids unnecessary delay of treatment and unnecessary additional radiation exposure. Bitewing radiographs within the past one year and Full Mouth Series or Panorex radiographs within the past five years are usually acceptable for clinical use. Additional radiographs may be taken if necessary. You may also print and use our Authorization of Dental Records and Radiographs Release form if needed. This form is available at the bottom of this page.
Scheduling
Our office is open Monday, Wednesday, and Friday from 9am-5pm by appointment.
If you have an emergency or are in pain we will make every effort to see you the same day. If this happens after business hours we always have a dentist on call who is available for emergency treatment or for referral to a dentist or specialist who can address your situation. Please call our main line, 978-232-9003, for after-hours emergency care.
If for any reason you are not able to make it for your appointment, please let us know at your earliest convenience. We appreciate notice of canceled appointment so we may attend to others who may be waiting for an appointment.
Here at BRAIDC, we strive to stay on schedule all the time, but unexpected delays do occur due to the many clinical challenges and dental emergencies we face. We thank you for your patience. Our ultimate goal is to provide the highest standard of care to all our patients with minimal waiting time.
Insurance and Financing
During your visit, we will be happy to contact your insurance company, verify coverage and provide you with any necessary financial information. We want you to know exactly what treatment is being proposed, what your insurance company may cover, and what your out-of-pocket estimate should be.
All professional services rendered are charged directly to the patient and patients are responsible for payment of fees. Payment is due at time of service. For your convenience, we accept most forms of payment; if you have dental insurance, we will be happy to submit a claim on your behalf.

Please double check with your insurance carrier to see if they provide out-of-network coverage. However, the coverage will not be as comprehensive as if you see an in-network provider. When you visit us, as we are an out-of-network provider, the insurance company still pays a portion of the fee, and you are responsible for the balance.

Please be sure to let us know what your insurance is, and if there have been any changes with your plan when you call to schedule with us. We are happy to contact your insurance company to verify coverage so that we can properly provide you with any necessary financial information.
We believe that quality dental care should be available to everyone. Beverly Restorative and Implant Dentistry Center is a preferred provider for Delta Dental and Blue Cross Blue Shield. We also submit to all major dental insurance companies. When you participate with us as an out-of-network provider, the insurance company still pays a portion of the fee, and you are responsible for the balance.
Financing
All professional services rendered are charged directly to the patient and patients are responsible for payment of fees. Payment is due at time of service. For your convenience, we accept most forms of payment; if you have dental insurance, we will be happy to submit a claim on your behalf.
We do not provide any payment plans, however if you are having any issues with payment, please do not hesitate to contact us and the office can see what we can do for you. Another option for payment is CareCredit, which does allow you to take advantage of short-term financing options in order to pay for treatment. For CareCredit we only accept 6 Month Deferred/No Interest plan for over $200 and 12 Month Deferred/No Interest plan for over $1000.
